This report is the customer's detail premium invoice reflecting the total amount due for the period selected. Included in this detail is a listing of the covered subscribers (ID number, name, coverage type and rate), summary of activity (ID number, name, activity type, effective date and amount) and past due itmes (invoice number, obligation number and amount).
